Support Group Information Form

Please use this form to give BCHEA updated information for your support group listing on our site. Give as much information as you can. You may use the “description” field to include the areas you cover, your religious affiliation, or other things you feel are important to share.

Starred fields are required in the event we need to contact either the person submitting this form or the contact person for the group.

BCHEA respects your privacy and won’t publish any information you indicate isn’t to be made public. Your group listing needn’t have anyone’s name, home phone number, or personal email address. A website, groups.io group, or Facebook group can be provided as well as a contact email for the group, if one exists.

 

Note: 

For your listing to be published on the website, we do require some form of contact information eg. one of website, facebook group, email or phone number.

Many support groups provide a google email address that reflects the name of the group and can be passed along as volunteers “graduate”. This helps keep your group’s information current wherever it might be listed and helps protect the privacy of your contact person.

Please do not send sensitive information (credit card numbers, etc.) through this form.
